Authorization Form

1. Authorization and Liability

I hereby authorize Five Star Autoplex (the "Company") to perform the repairs described in the estimate, including the procurement of all necessary materials. I acknowledge that:

• The Company is not liable for loss or damage to the vehicle—or personal items left resulting from fire, theft, or any cause beyond its control.
• The Company is not responsible for delays caused by parts availability or transportation issues.

Company employees are allowed to use the vehicle on public roads or other locations for testing, calibration, and inspection purposes.

An express mechanic’s lien ensures that payment will be received for repairs performed.

2. Parts and Repair Standards

Original Equipment Manufacturer (OEM), aftermarket, or recycled parts can be used for repairs or replacements.

The determination to repair or replace a component is at the sole discretion of the repair technician to ensure safety and structural integrity. The insurance company will choose type of parts.

3. Payment, Repossession, and Completion

• Notice Pursuant to Texas Property Code §70.001: As the person or authorized agent obligated to pay for these repairs, I understand this vehicle is subject to repossession under Texas Business and Commerce Code §9.609 (formerly §9.503) if payment is stopped, dishonored for insufficient funds, or drawn on a closed account.
• Estimates: Completion dates are estimates only. Delays due to insurance re-inspections, supplemental/hidden damage, or parts backorders are unpredictable and may extend the repair timeline.

Vehicle Release: A vehicle will only be released once full payment has been received or proof of payment has been verified.

4. Limited Power of Attorney

KNOW ALL MEN BY THESE PRESENTS: I (or we) hereby appoint Five Star Autoplex as my lawful agent and attorney-in-fact to endorse my name on any check or draft issued by all Insurance, warranty, and payment Companies for the purpose of settling the repair costs for the vehicle listed herein. I hereby ratify and confirm all actions taken by said agent in accordance with this authorization.

Contract Termination and Total Loss Fees

In the event of contract termination or if the vehicle is deemed a "Total Loss" by an insurance provider, the following charges will apply and must be settled before the vehicle is released:

In the event of a contract termination, or should the vehicle be declared a total loss by an insurance provider, the following fees shall become due and payable prior to the release of the vehicle:

• Administrative Fee: A flat fee for file processing and documentation.
• Labor & Tear Down: Charges for any disassembly, diagnostic work, or labor performed to date.
• Storage & Logistics: Daily storage fees and applicable towing or recovery charges.
• Termination Fee: A fee equal to 35% of the Total Repair Estimate to cover procurement, handling, and loss of scheduled shop production.
